  • 2-101.11

2-101.11 Assignment (Pf) 2-102.11 (A),(B) and (C)(1),(4)-(16)Demonstration (Pf) 2-103.11 Person-In-Charge [PIC] Duties (Pf) The facility remains on Long-Term Corrective Action (LTCA) for recurring violations involving cold holding, food storage, cross-contamination, and PIC responsibilities. During this inspection, all three reach-in refrigerators were found above 41°F ambient temperature, with individual food items also measuring above 41°F. The PIC explained that refrigeration capacity was limited because other refrigeration units and freezers were full, preventing relocation of food. EHS reminded the PIC that adequate cold-holding capacity had been addressed during the LTCA meeting and that foods held out of temperature for the applicable period, including items dated 9/9/26 and prior, would need to be discarded. The PIC stated that the refrigeration temperatures could be lowered, and staff were observed cleaning the refrigeration motors and adjusting the temperature settings. After approximately 45 minutes, two of the three units were within the appropriate temperature range, with randomly selected food items measuring 41°F. The PIC agreed to discard items that remained out of temperature and to maintain ambient thermometers inside each refrigeration unit. The recurrence of these deficiencies while under LTCA indicates a continued lack of sustained active managerial control and demonstrates that the PIC has not consistently ensured adequate monitoring, corrective action, and prevention of previously identified food safety hazards.

  • 4-601.11

4-601.11(A) Equipment, Food-Contact Surfaces, Nonfood-Contact Surfaces, and Utensils (Pf) Observed a significant accumulation of mold-like substance/growth along the entire seal of the walk-in refrigerator. This could also reflect on the issue of improper cold holding temperature in the walk-in refrigerator. EHS informed PIC cleaning frequency would have to be increased to control for further growth.
