  6-202.13 Insect Control Devices, Design and Installation (C) Noticed 1 large spring-loaded rat trap and 1 large rat motel near water heater and back door that was not well sealed. 6-202.15 Outer Openings, Protected (C) Observed rear door not properly sealed to prohibit insects and other animals. 6-202.16 Exterior Walls and Roofs, Protective Barrier (C) Observed numerous unsealed holes in walls and roof, aparantly cut for electrical or plumbing--these should be foamed. 6-501.111 Controlling Pests (Pf, C) Observed a significant harborage issue with respect to accumultated grease and spilled / leaked syrup. 6-501.115 Prohibiting Animals (Pf) Observed a general deterioration of infrastructure i.e, floor tile, coving, walls, ceiling (above hot water heater), walk-in floor, both inside and out-rusted through and leaking--all areas that trap food debris and contribut to attracting animals. There is significant dead grasshopper litter and buzzing lly problem. PIC is aware and advised owner is aware. Advised PIC that this inspection grade will mandate me to return in approximately two weeks to re-inspect and all violations must be corrected at that time. PIC understands. EHS provided business card for owner to call if Qs arise.
