  Plumbing Installed; Proper Backflow Devices

  Replace the hoses from the drains and overflow drain of the ice machine with rigid PVC or ABS pipes. Drainpipes must be rigid and indirectly discharge to the floor sink with minimum 1" air gap from the finished floor. Must repair all plumbing to a state of good repair prior to next routine inspection.

  Physical Facilities Installed, Maintained, and Clean

  Noted the restrooms and new server area were not provided with approved 3/8" radius cove base at walls and floor junctures or cabinet base and floor junctures. Please provide approved 3/8" radius cove base. Straight base is not acceptable in restrooms, dry storage, work zones or kitchen. All floors, walls, and ceilings must be maintained and repaired such that it is smooth and easily cleanable. Repair physical facility prior to next routine inspection.

  Noted black ceiling tiles over the bar and server areas. Please replace/ paint with semi-gloss paint with light color ceiling tiles.
