During this inspection observed all 3 floor drains in back of house missing drain grates/drain baskets. Ensure all floor drains have dome grates/drain baskets. Corrective Action: Correct prior to next routine inspection.

During this inspection observed again men's restroom door not able to self close fully. Notified PIC. ====A toilet room located on the premises shall be completely enclosed and provided with a tight fitting and self-closing door. Corrective Action: Correct prior to next routine inspection. 5-501.17 - Core: Toilet Room Receptacle, Covered Addl Notes: On previous inspection advised Person In Charge to add trash can with lid in women's restroom. During this inspection again observed no lid on trash can. Notified Person In Charge (PIC).===A toilet room used by females shall be provided with a covered receptacle for sanitary napkins Corrective Action: Correct prior to next routine inspection.
