  6-101.11 Surface Characteristics-Indoor Areas (C) Observed significant accumulation of food debris, congealed fryer oil and lent in corners and at floor-wall junctures. Observed missing grout and floor tiles such that the depressions are collecting water and growing slime-mold-like substance', in several locations throughout. Missing coving and holes in walls. 6-201.13 Floor and Wall Junctures, Coved, and Enclosed or Sealed (C) 6-501.12 Cleaning, Frequency and Restrictions (C) PIC advised that ICE Machine is cleaned by GM aproximately every 2 weeks. EHS extracted air-filters that were completely obstructed. Interior bin appeared clean but the actual machine could not be inspected. Observed an out-of-service fountain machine in the back area that haad splattered syrup splattered splattered. Please clean or remove if non-functioning. Please write an SOP for more frequent deep cleaing of facility, especailly in hard to reach places. 6-501.114 Maintaining Premises, Unnecessary Items and Litter (C) Observed significant lent and grime on top of all machinery. PIC will correct.
