Walk in cooler/walk in freezer Observed, within the main walk in cooler where the walk in freezer is accessed through another door, a paper sign to be taped to the freezer door. Observed black mold-like substance on the paper, which also appeared to be wet/dried/in place for a significant amount of time. The area around this observation was noted to have a mold-like/mildew-like smell.

Walk in cooler, dish room Observed accumulation of debris/residue on the floor of the walk in cooler, particularly under shelving. Observed black mold-like substance on the underside of shelving above the 3 comp sink; also observed accumulation of debris/dust on the dish machine vent hood (interior) and on top of the dish machine.
