Addl Notes: Observed 0ppm of quat sanitizer in three compartment sink behind service counter with bucket submerged in solution to be sanitized. All quat sanitizers must have a concentration of 150-400ppm to ensure food contact surfaces are properly sanitized. Sanitizer bag was changed a concentration of 200ppm was achieved. Corrective Action: Corrected at time of inspection.

PRIORITY VIOLATION-5-205.12 (A) - Priority: Prohibiting a Cross Connection; Drinking Lines Addl Notes: Observed sprayer nozzle attached to hose downstream of atmospheric vacuum breaker (AVB) with faucet turned on. Shut-offs, including chemical dispensers and sprayer nozzles, are not approved for continuous installation downstream from atmospheric vacuum breakers. Sprayer nozzle was removed at time of inspection. Corrective Action: Corrected at time of inspection.

All flooring must be smooth and easily cleanable. According to manager floors are scheduled to be refinished. Corrective Action: Correct prior to next routine inspection.
