Discussed with PIC that covered waste receptacle is required in unisex/female restrooms. Predefined Comment: Uni-sex and female toilet rooms require a covered waste receptacle for sanitary napkins. Provide prior to next routine inspection. Corrective Action: Correct prior to next routine inspection.

Discussed with PIC that garbage/refuse required to be covered at all times. Predefined Comments: Outside receptacles must be closed with tight fitting lids or doors. 2nd consecutive occurrence Corrective Action: Correct prior to next routine inspection. 5-501.115 - Core: Maintaining Refuse Areas and Enclosures Addl Notes: Observed dumpsters overflowing with cardboard boxes and garbage. Discussed with PIC that garbage refuse areas must be maintained at all times. Predefined Comments: Storage area and enclosure shall be maintained free of unnecessary items and clean. Corrective Action: Correct prior to next routine inspection.

6-501.12 - Core: Cleaning, Frequency and Restrictions Addl Notes: Observed dirt build up on walls in hallway of restrooms, on restroom doors, backroom swing doors and office door. Discussed with PIC that physical facility required to be cleaned more frequently to prevent accumulation of organic matter, dirt, dust and food debris. Predefined Comment: Physical facility must be cleaned frequently enough to prevent accumulation of dirt, organic matter, and food debris. Must clean physical facility prior to next routine inspection. 3rd consecutive occurrence Corrective Action: Correct prior to next routine inspection.
