Escutcheon missing from drain pipe entering wall. Predefined Comment: Physical facility must be maintained in good repair at all times. A couple areas of the ceiling also observed with leaks during rain. Repair physical facility prior to next routine inspection. Corrective Action: Correct prior to next routine inspection.

Sanitizer will be made manually until dispenser is repaired. No sanitizer was in use at time of inspection. Predefined Comment: Must repair all plumbing to a state of good repair prior to next routine inspection. Corrective Action: Correct prior to next routine inspection.
