PRIORITY VIOLATION-5-202.13 - Priority: Backflow Prevention, Air Gap Addl Notes: Observed on previous inspection and notified Person In Charge and reviewed and provided Maricopa County backflow guidance documents. On this current inspection observed backflow had no air gap to rpz drain. Notified Person In Charge (PIC). PIC will place work order. ===Air gap is less than 1 inch. Corrective Action: Correct prior to re-inspection. 5-205.12 (B) - Priority Foundation: Prohibiting a Cross Connection; Non-Drinking Lines Addl Notes: Observed on previous inspection AVB attached to chemical dispenser system and left on. Reviewed and provided guidance documents on proper usage of chemical dispenser. On this inspection observed the same issue again with faucet at mop sink with avb installed and directly connected to chemical dispenser unit and left on water. Notified Person In Charge (PIC). PIC had employee disconnect cross connection.===AVB with a Y-valve attached with a direct supply water line feeding a chemical dispenser. Corrective Action: Corrected at time of inspection.

PIC will restock toilet paper at men's restroom. ===A supply of toilet tissue shall be available at each toilet. Corrective Action: Corrected at time of inspection.
