6-501.12 (A) PHYSICAL FACILITIES shall be cleaned as often as necessary to keep them clean. Flooring near mop sink and flooring under 3 comp sink Observed accumulation of debris, soils and residue on the flooring in these areas. Also beneath the 3 comp sink an old rotting produce item was observed. Facility is proactively abating a known pest issue.
