2-102.12 (A) At least one EMPLOYEE that has supervisory and management responsibility and the authority to direct and control FOOD preparation and service shall be a certified FOOD protection manager who has shown proficiency of required information through passing a test that is part of an ACCREDITED PROGRAM. Facility Observed that the only Serv Safe Certificate at the facility is expired. The PIC stated that this certificate belonged to the only manager and that his certificate is currently at his old store.

4-501.116 Concentration of the SANITIZING solution shall be accurately determined by using a test kit or other device. Facility Observed that the facility uses quaternary ammonium compound test strips. Found three containers, one which expired in 2024, one in 2025, and one Jan of 2026.
