4-501.114 A chemical SANITIZER used in a SANITIZING solution for a manual or mechanical operation at contact times specified under 4-703.11(C) shall meet the criteria specified under §7-204.11 Sanitizers, Criteria, shall be used in accordance with the EPA-registered label use instructions, and shall be used as follows: (C) A quaternary ammonium compound solution shall: (1) Have a minimum temperature of 24°C (75°F), (2) Have a concentration as specified under § 7-204.11 and as indicated by the manufacturer's use directions included in the labeling, and (3) Be used only in water with 500 MG/L hardness or less or in water having a hardness no greater than specified by the EPA-registered label use instructions; 3 compartment sink Observed wash and sanitize basins filled with solutions and dishes on the drain board appearing to have recently been placed to dry. Observed quat sanitizer dispenser mounted on wall and a container of bleach (chlorine) on the floor. Measured both quat and chlorine at 0 ppm within the sanitize basin. When asked, PIC stated the employees likely would have used the dispenser for the quat; also stated it might be an old solution that needs changed out. Tested the solution coming directly from the dispenser at 0 ppm quat after running for several seconds. Noted the line from the sanitizer container to the dispenser was detached.

5-203.14 A PLUMBING SYSTEM shall be installed to preclude backflow of a solid, liquid, or gas contaminant into the water supply system at each point of use at the FOOD ESTABLISHMENT, including on a hose bibb if a hose is attached or on a hose bibb if a hose is not attached and backflow prevention is required by LAW, by: (A) Providing an air gap as specified under § 5-202.13 ; 3 compartment sink Observed the spray nozzle hose to be broken allowing the supply end of the nozzle to hang down almost touching the bottom of the rinse basin on the 3 compartment sink.
