6-202.14 Except where a toilet room is located outside a FOOD ESTABLISHMENT and does not open directly into the FOOD ESTABLISHMENT such as a toilet room that is provided by the management of a shopping mall, a toilet room located on the PREMISES shall be completely enclosed and provided with a tight-fitting and self-closing door. The handle to close the men's room is completely gone/missing, there is a gaping hole in the door. Observed men's restroom door to have a handle however womens restroom is not self-closing- broken door attachment. Facility shall ensure that restrooms have self-closing doors and are in good repair. This is a repeat violation.

6-501.11 PHYSICAL FACILITIES shall be maintained in good repair. There are several cracked tiles around a drain in the back room. The urinal is covered/out of service. Observed cracked tiles by soda syrup. Facility shall ensure that physical facilities are maintained in good repair. Correct violation by next routine inspection. This is a repeat violation.
