  7-202.12 POISONOUS OR TOXIC MATERIALS shall be: (A) Used according to: (2) Manufacturer's use directions included in labeling, and, for a pesticide, manufacturer's label instructions that state that use is allowed in a FOOD ESTABLISHMENT, Facility Observed a bottle of Raid underneath a handwashing sink that stated "Indoor Residential Use Only" on the label. The PIC stated that the chemical was used to help with the cockroaches.

  6-501.111 The PREMISES shall be maintained free of insects, rodents, and other pests. The presence of insects, rodents, and other pests shall be controlled to eliminate their presence on the PREMISES by: (B) Routinely inspecting the PREMISES for evidence of pests; Facility Observed several living German cockroaches around the handwashing sink next to the grease trap. The PIC stated that they are working with the pest control company AZ Pest Control to manage their cockroach issue, who comes by to apply chemicals monthly. Observed that the facility was kept clean, plumbing was in good repair, and no food was being stored on the floor. Did not observe any cockroaches on any food-contact surfaces.

  3-304.14 (B) Cloths in-use for wiping counters and other EQUIPMENT surfaces shall be: (1) Held between uses in a chemical sanitizer solution at a concentration specified under § 4-501.114; and (2) Laundered daily as specified under 4-802.11(D). Sanitizer Bucket Observed Sushi Ginger buckets being reused as a sanitizer bucket. The PIC stated that he adds bleach to create a solution that he uses to clean the cutting boards and other food contact surfaces. Observed that the facility does not have any chlorine test strips. Using chlorine test strips provided by the inspector, observed that the concentration of chlorine in the solution was over 200 ppm.

  4-501.116 Concentration of the SANITIZING solution shall be accurately determined by using a test kit or other device. Facility Observed that the test strips above the three compartment sink verifying the amount of quaternary ammonium was ineffectual (the color on the indicator did not change). Observed that the condition of the strips was poor, as it was discolored and covered with what appeared to be cockroach feces. Observed that the facility uses chlorine to make their sanitizing buckets but did not have any chlorine test strips onsite to verify concentration. Using quaternary ammonium test strips, verified that the concentration of sanitizing solution in the three compartment sink was below 150 ppm. Using chlorine test strips, verified that the concentration of bleach in the sanitizing buckets was over 200 ppm.
