PIC stated that the facility had employees sign Form 1-B, but was not able to present verification of signed Form 1-B from each employee during permit renewal inspection.

  2-102.12 (A) At least one EMPLOYEE that has supervisory and management responsibility and the authority to direct and control FOOD preparation and service shall be a certified FOOD protection manager who has shown proficiency of required information through passing a test that is part of an ACCREDITED PROGRAM. Upon permit renewal inspection, Person-in-Charge (PIC) or food employee with supervisory/managerial authority did not have Food Protection manager certification. Facility shall ensure at least one employee with supervisory or managerial authority has certification. Correct by next routine inspection.
