  2.201.11(C) The PERSON IN CHARGE shall ensure that a CONDITIONAL EMPLOYEE: (1) Who exhibits or reports a symptom, or who reports a diagnosed illness as specified under Subparagraphs (A)(1) - (3) of this section, is prohibited from becoming a FOOD EMPLOYEE until the CONDITIONAL EMPLOYEE meets the criteria for the specific symptoms or diagnosed illness as specified under § 2-201.13;P and (2) Who will work as a FOOD EMPLOYEE in a FOOD ESTABLISHMENT that serves as a HIGHLY SUSCEPTIBLE POPULATION and reports a history of exposure as specified under Subparagraphs (A)(4) – (5), is prohibited from becoming a FOOD EMPLOYEE until the CONDITIONAL EMPLOYEE meets the criteria as specified under 2-201.13(I). The Person In Charge (PIC) was unaware of the Employee Illness Policy requirement. The Person In Charge stated that they do not have a current Employee Illness policy training nor signed Form 1-B's. The Person In Charge stated that a poster with the reportable signs and symptoms would be ordered.

  6-501.112 Dead or trapped birds, insects, rodents, and other pests shall be removed from control devices and the PREMISES at a frequency that prevents their accumulation, decomposition, or the attraction of pests. Storage Area, Office Entrance, Entrance to the Building, Entrance to Refrigerator Observed multiple dead insects and one dead cockroach. Did not observe any live insects. The PIC stated that the dead bugs are removed every morning and that they receive routine pest control services monthly.
