Observed build ice buildup on the fan guard in reach in freezer. Observed buildup on the sprayer at the 3 compartment sink. Person in charge cleaned sprayer at time of inspection.

Observed a container of coleslaw not marked to indicate the date or day by which the food shall be consumed, sold or discarded. Person in charge marked coleslaw at time of inspection.

Physical facilities not maintained in good repair & cleaned as often as necessary to keep them clean

Observed buildup on the restroom door. Observed buildup and debris on floor though out back room and restroom. Observed buildup on the floor near back door.
