Employee accommodations not clearly designated for eating/drinking/smoking

Observed employee drinks on the main cook prep table.

Insect control devices do not retain insect within device and/or are not properly installed

Observed sticky fly traps on the floor at the end of the bar.

Equipment not maintained free of encrusted grease/soil deposits

Observed build up on the hood vents. Observed excessive grease build up on the back of the fryers. Repeat violation will be assessed in 3-10 days.

Physical facilities not maintained in good repair & cleaned as often as necessary to keep them clean

Observed build up on the floors main kitchen under reach in coolers and cooking equipment. Observed a hole in the wall by the back reach in cooler.

Equipment & Food-Contact Cleaning Frequency - Equipment food-contact surfaces and utensils shall be cleaned according to the Food Code.

Observed build up on the can opener blade. Observed build up on the ice chute of the basement ice machine.

Hot Water and Chemical - After being cleaned, equipment food-contact surfaces and utensils are not sanitized in accordance with the Food Code.

Observed the chlorine dish machine to be at 0ppm in the main kitchen. Repeat Violation will be assessed in 3-10 days.
