  Violation of Code: [511-6-1.05(6)(a)] Observed reach-in cooler at 54F. At time of inspection, there were no TCS food items in the cooler. CA: Advised PIC equipment shall be maintained in a state of repair and to ensure no TCS foods are stored in that cooler until it is repaired or replaced. New Violation.

  Violation of Code: [511-6-1.05(7)(d)] Observed build up of food debris on non-food contact surfaces. CA: Advised PIC nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residue. New Violation.

  Violation of Code: [511-6-1.07(5)(a),(b)] Observed walls and ceilings with debris buildup in the main kitchen. Observed ceiling tile in disrepair in dish washing area. CA: Advised PIC the physical facilities shall be maintained in good repair and cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products. Repeat Violation.
