Food Service Establishment Pre-opening inspection in response to a change of ownership. LICENSE IS APPROVED. Scope of business: Establishment intends to prepare pizza and sides for dine in and carry out service. Establishment is equipped to cook, reheat, hot/cold hold and cool. Extensive changes to the menu or remodeling of the establishment require prior approval by the Regulatory Authority. Certified Food Protection Manager: Pre-opening inspection. Person In Charge understands an employee designated as a person in charge shall obtain their certified food protection manager certification no later than 12/29/2026. Facility to share certification documentation with inspector upon course completion. Firm has been made aware of the basic food code requirements applicable to their establishment and current operation. Person In Charge understands the responsibility of the permit holder to comply with those requirements. If a subsequent on-site inspection reveals non-compliance with regulations, the permit holder agrees to bring violations into full compliance within the time frame specified in the Iowa Food Code. A routine inspection will be scheduled in 90-120 days.
