Food Service Establishment Pre-Opening Inspection Conducted on 08/19/2026. LICENSE IS APPROVED. Scope of Business: Establishment intends to sell chicken sandwiches, mac & cheese, cookies, and desserts on-site at the Amazon fulfillment center in Davenport, IA. All items are prepared and pre-packaged from the North Park Mall location. Food items are kept in thermal bags to ensure proper hot holding. Serving times are typically limited to 1-2 hours. Extensive changes to the menu or operation requires prior approval by the Scott County Health Department. Limitations: Due to there being no cooking equipment, cooking on-site is not permitted. There is not a specific hand washing sink in the area where they are set up, serving food is also not permitted. Certified Food Protection Manager (CFPM): The North Park Chick-Fil-A has several certified food protection managers on file. Firm has been made aware of the basic food code requirements applicable to their establishment and current operation. Person in charge understands the responsibility of the permit holder to comply with those requirements. If a subsequent on-site inspection reveals non-compliance with regulations, the permit holder agrees to bring violations into full compliance within the time-frame specified in the Iowa Food Code.
