A complaint investigation and a routine inspection were each conducted at the facility on site on 11/26/2025. Complaint was received about a roach being observed in the front area by a customer. Upon investigation, live roaches were observed in the front customer and serving area. Facility does have pest control and visited the facility in November 2025. Complaint is verifiable. Clean and sanitize the facility, repair the pipes and the counter, cover food, and do not allow moisture to accumulate. The following violations were observed during the inspection: Item #10- Bucket of water stored in front hand sink. Paper towels missing from hand sink. Battery not working on the men's restroom paper towel dispenser. Bucket of water was removed from the hand sink. Paper towels were provided to each hand sink. Battery was replaced for the men's restroom paper towel dispenser. Violation was corrected. Item #15- Walk in freezer has a major leak and has major ice build up. This is a repeat violation. Facility has to repair this issue or replace the walk in freezer. Item #28- Spray bottle of bleach in the front area was too powerful indicated by the test strip. Water was added to the spray bottle, retested with a new test strip, and the chlorine concentration was approximately 100ppm. Violation was corrected. Item #38- Live roaches were observed in the front area by the drain and the ice cream freezer. Facility will eliminate the roaches. Contact pest control. Clean and sanitize the facility, repair the pipes and the counter, cover food, and do not allow moisture to accumulate. Item #44- Utensils were stored in the dipper well but the water was not running. Water was turned on and the water flow was strong enough and the violation was corrected. Item #47- The walk in cooler door seal and the right door seal of the MasterBilt Retail Freezer were each in poor condition. Facility will replace the door seals. Item #49- Debris and gunk present near the floor drain, on the counter shelves, and near the ice cream freezer. Facility will clean and sanitize all areas. Item #51- The 3-compartment sink faucet does not have water that shuts off from the hot and cold water handles. The outlet pipe under the 3-compartment sink leaks and is propped up by a bucket. The dipper well in the front serving area is not functioning properly. Facility will repair all plumbing issues. Item #55- The front counter is in very poor condition. This is a repeat violation. Facility will repair the counter.
