A complaint investigation and a routine inspection were each conducted at the facility on site on 12/05/2025. Complaint was received by a customer about sanitation and cleanliness in the facility. Numerous areas are in need of cleaning and sanitizing in the facility. Dust buildup, debris, and gunk are present in many areas of the facility. Complaint is verifiable. The following violations were either corrected on site or are in further need of correction: Item #8- Employees were not washing their hands in between doing different tasks. Employees were using their cell phones then went to the grill to cook customers' food without hand washing. Employees washed their hands. Employees must wash their hands often. Violation was corrected on site. Item #10- Back hand sink had items stored inside of the sink. Front hand sink had ice dumped into it. Items were removed from the back hand sink. Front hand sink had the ice melted from it. This is a repeat violation. Person in charge will make sure hand sinks are accessible at all times and that hand sinks are used for hand washing only. Violation was corrected on site. Item #13- Soy sauce was being stored at room temperature when the label states "refrigerate after opening." Soy sauce was disposed of. Monitor labels and the violation was corrected on site. Item #15- Raw pork stored above carrots in the walk in cooler. Raw pork was moved to the bottom shelf away from and below ready to eat items in order to prevent contamination. Several food items uncovered. Bottom of containers stored in direct contact with food below the containers. This is a repeat violation. Person in charge will make sure all raw items are stored below ready to eat foods and will make sure all food is covered and protected. Item #16- Numerous areas with debris and gunk: knives, knife rack, ice machine, fountain drink machine, walk in cooler racks, and utensils. Utensils are only cleaned and sanitized twice a day but that frequency is not enough based on the hours of operation. This is a repeat violation. Person in charge will make sure that all areas are cleaned and sanitized and will make sure utensils are cleaned and sanitized at much higher frequency. Item #23- Multiple food products missing labeling (prep dates). This is a repeat violation. Person in charge will make sure that all prepared foods and all meats are labeled. Item #28- Spray bottle missing label of the contents that are in the bottle. Windex stored with food and beverages in the front waitstaff area. Spray bottle was labeled with the name of the contents in the bottle. Windex was moved away from the food and beverages in order to prevent contamination. This is a repeat violation. Person in charge will make sure that all spray bottles are labeled and will make sure all cleaners and toxic items are stored away from food, beverages, and contact items. Item #41- Plain water used to sanitize utensils and surfaces and not sanitizer and water. Facility will provide sanitizer to the water in order to sanitize areas properly. Item #43- Bowls used to disperse food in the kitchen. Facility will use scoops with handles. Item #45- Trays presented with contact surfaces in the kitchen. Facility will invert all trays. Item #49- Walls, ceilings, racks, and table legs in the kitchen with debris and gunk. Walls and ceilings in the restrooms with dust present. Facility will clean and sanitize all areas. A recheck inspection will occur on or after 12/15/2025. Follow-up will be completed on or after 12/15/2025 by Physical Recheck.
