Routine inspection conducted on 11/05/2025 due to a non-illness complaint. The facility has an employee illness reporting agreement policy and two certified food protection managers. Complaint is VERIFIED. Complainant contacted SCHD with concerns over pest control. Items that require additional action: #5- Facility manager agrees to locate or draft a bodily fluid cleanup procedure. #16- Facility manager agrees to perform additional cleaning on the items mentioned above, as well as provide additional training and oversight on washing, rinsing, and sanitizing in-use utensils. #38- Facility manager agrees to contact area manager and general manager about pest control issues, as well as clean-up drippings found throughout. #44- Facility manager agrees to find a spot for pizza dough basins, so they are stored at least 6 inches from the floor. #55- Facility manager agrees to perform additional cleaning on the flooring throughout. Physical facilities are in desperate need of repair, which was mentioned during the previous routine inspection (12/06/2023), and it had been noted on (03/28/2024) that the facility had plans to repair the flooring by the next routine inspection following replacement of the large pizza oven. These repairs had not been completed. The facility manager mentioned the facility is due for a re-model in the next few months, per requirement of corporate, but the facility is required to submit formal plans for remodel and provide the inspector with a guaranteed timeline of when the work is to be completed. If the facility fails to make repairs in a timely manor, the Scott County Health Department will consult with the Scott County attorney on further action, which could include license suspension or closure. The health dept. will return on or after 11/15/2025 for a physical re-inspection. Any questions or follow-up can be sent to Raymond.Miller@scottcountyiowa.gov Follow-up will be completed on or after 11/15/2025 by Physical Recheck.
