Conducted pre-opening inspection of facility today. LICENSE IS APPROVED. Scope of Business: Establishment intends to offer hot and cold food items like pizza, breadsticks, egg rolls, ice cream cups, etc. during late afternoon and evening hours. Delivery service from a third party may also be offered. Establishment is equipped to cook, reheat, hot/cold hold, and cool. Extensive changes to the menu or remodeling of the establishment require prior approval by the Regulatory Authority. Certified Food Protection Manager: Facility has an active Certified Food Protection Manager on staff. Firm has been made aware of the basic food code requirements applicable to their establishment and current operation. Person in charge understands the responsibility of the permit holder to comply with those requirements. If a subsequent on-site inspection reveals non-compliance with regulations, the permit holder agrees to bring violations into full compliance within the time-frame specified in the Iowa Food Code. Facility has an Employee Reporting Agreement Policy in place, and a written, established procedure available for responding to the clean-up of bodily fluids. Facility will send proof of correction for item #38 to inspector at: tara.marriott@scottcountyiowa.gov by 9/27/2026.
