Routine inspection conducted with Non Illness Complaint alleging food from unapproved sources, pest control, cleanliness & Illness Complaint alleging symptoms after eating. Facility was not aware of either complaint. Stated no staff reporting ill or excluded based on the conditional food employee health training form they demonstrated. Stated no water or power outages in prior 30 days. Walk in cooler was not maintaining tempurature for cold hold at inspection. Facility does not use regular pest control and had not reports to demonstrate though stated they have a licensed local vendor that they call or would call if they noticed an issue. Stated no food is used in restaurant from unlicensed sources. Complaints Closed Unverifiable. 10. Hand sink signage stating hand wash only is attached and will be verified at recheck. 33. Facility agrees to use other reach in coolers that are available not to use the walk in cooler for storage of tempurature controlled foods until repaired. Inspector will verify repair at recheck. 57. Person in charge agrees to renew their food license within 5 business days. They understand that they must communicate with the DIA office to ensure all penalty fees are paid at the time of renewal. Follow-up will be completed on or after 8/20/2026 by Physical Recheck.
