This was a Routine inspection conducted as a result of an illness complaint received by the Iowa Department of Inspections and Appeals & Licensing. Owner was not aware of this specific complaint. Complaint is closed and unverifiable. The following item is a repeat risk factor violation. Owner will implement the following long term corrective action: 3. Training will take place on symptoms associated with foodborne risk factors. Owner stated he will utilize form 1-B. 23. Owner stated she will use guidance documents to train new staff on datemarking. 28. Owner stated she will train new staff to label cleaning products. Follow-up will be completed on or after 12/12/2025 by Physical Recheck.
