OBSERVED INDIVIDUAL ACTIVELY SMOKING IN FACILITY UPON ARRIVAL. OPERATOR QUICKLY REMOVED ASHTRAY TO SPACE UNDER COUNTER. WHEN ASHTRAY WAS PULLED OUT OF SPACE, THE OPERATOR QUICKLY GRABBED ASHTRAY AND REFUSED TO LET INSPECTOR EXAMINE FURTHER. INSTRUCTED MANAGER TO ENSURE COMPLIANCE WITH CLEAN INDOOR AIR ORDINANCE. 7-32-015.
