Observed spray bottle with liquid on front paper towel dispenser to not have common name label. person in charge provided label at time of inspection.

Openings to outside not protected against entry of pest as specified in the Food Code; and/or Protective barriers not provided for exterior walls/roofs

Observed light gap between front double doors of establishment. Observed light gap in bottom right corner of back exit door of establishment.

Physical facilities not maintained in good repair & cleaned as often as necessary to keep them clean

Observed stained ceiling tile in left corner of office. Observed build up on ceiling vent in both bathrooms.
