  Upon acceptance of the permit issued by the regulatory authority, the permit holder in order to retain the permit shall: Post the permit in a location in the food establishment that is conspicuous to Consumers; Mount permanent business name to the outside of the establishment in a conspicuous location

  Observed food permit to not be posted in a location conspicuous to customers. Person in charge moved permit at time of inspection.

  Observed 12 packs of Lunch Makers loaded nachos to be held past use by date of 7/13/26. Person in charge removed at time of inspection.

  Observed ice build up behind top of 5 door from the right of the reach in freezers. Observed ice build up on the right strips of walk-in freezer strip curtain.
