  Observed ice build up under pipe behind cooling unit in walk-in freezer. Observed build up on floor under equipment throughout main kitchen. Observed build up on wall behind fryers. Observed build up on shelf and floor inside 1 door low boy.

  Observed light gap under left door of main double door entrance.
