Before Use After Cleaning - Utensils and food-contact surfaces of equipment are not sanitized before use after cleaning.

Observed no sanitizer buckets to be made in kitchen or prep area. Person in charge made quat sanitizer bucket to be between 200-400 ppm at time of inspection.

Effective pest control measures not in place

Observed gnats throughout kitchen and prep areas.

Removing Dead or Trapped Birds, Insects, Rodents, and Other Pests.

Observed dead roach on the floor under coffee and tea machines.

Single-service/use items not stored as specified in the Food Code

Observed to-go boxes stored on the floor in the dry storage area. Person in charge removed to-go boxes from the floor at time of inspection.

Food is not stored as required by the Food Code

Observed frosted flakes and box of ketchup packets stored on the floor in the dry storage area. Food employee removed frosted flakes and box of ketchup packets from the floor at time of inspection. Observed box of chicken stored on the floor of walk in cooler. Food employee removed box of chicken from the floor of walk in cooler at time of inspection. Observed box of bread, burger patties, and hush puppies on the floor in the walk in freezer. Food employee removed box of bread, burger patties, and hush puppies from the floor at time of inspection.

Equipment & Food-Contact Cleaning Frequency - Equipment food-contact surfaces and utensils shall be cleaned according to the Food Code.

Observed build up on the diffuser of tea machine. Person in charge removed diffuser of tea machine to be washed, rinsed and sanitized at time of inspection.

Package Integrity - Food packages were not in good condition and did not appear to protect the integrity of the contents so that the food is not exposed to adulteration or potential contaminants.

Observed 5 dented cans of black beans. Person in charge dented cans from shelf to be returned at time of inspection.

Using a Handwashing Sink - A handwashing sink is not maintained so that it is accessible at all times for employee use; is being used for purposes other than handwashing or is not being used in accordance with manufacturer's instructions

Observed hand sink across from dry storage to be blocked by empty boxes. Person in charge removed empty boxes from blocking hand sink at time of inspection.

Handwashing Cleanser, Availability - Each handwashing sink or group of 2 adjacent sinks is not provided with a supply of hand cleaning liquid, powder, or bar soap.

Observed no hand soap in the men's restroom.

Hand Drying Provision - Each handwashing sink, or group of adjacent sinks, is not provided with Individual, disposable towels; a continuous towel system that supplies the user with a clean towel; a heated-air hand drying device; or a hand drying device that employs an air-knife system that delivers high velocity, pressurized air at ambient temperatures.

Observed no paper towels at hand sink next to ice machine.

Toilet rooms & fixtures not cleaned as often as necessary

Observed men and women restroom & fixtures not cleaned.

Maintaining & Using Handwashing Sinks - Handwashing sinks are not kept clean, maintained and/or used as specified in the Food Code.

Observed reddish brown build up in hand sink next to ice machine.

Nonfood-contact surfaces not cleaned at frequency to prevent buildup of residue

Observed red build up at the bottom of 2 door reach in cooler. Observed grease build up on the sides of grill, bread box, stove and deep fryer.

Physical facilities not maintained in good repair & cleaned as often as necessary to keep them clean

Observed build up and debris under racks and shelves in the dry storage area. Observed hole in the wall above ice machine. Observed debris under shelving unit along the walls in the dishwasher area. Observed build and debris under dishwasher. Observed brown and black build up along the metal at dishwasher. Observed stained ceiling in dishwashing room. Observed pink and grey slime build up coming the drain and on to the grates of the drain in front of the steamwell.

Equipment not in good repair & adjusted per manufacturer's specs

Observed air conditioning unit in the dishwasher area to be in disrepair and broken with wires exposed.
