Observed a towel in the hand sink. Person in charge removed towel at time of inspection.

Common Name, Working Containers - Working containers used for storing poisonous or toxic materials such as cleaners and sanitizers taken from bulk supplies are not clearly and individually identified with the common name of the material.

Observed a common name spray bottle not labeled. Person in charge labeled spray bottle at time of inspection.

Dressing rooms not designated if employees change on-site; and/or Lockers not provided for employee belongings

Observed a purse stored near clean utensils. Person in charge moved purse to designated area.

Employee accommodations not clearly designated for eating/drinking/smoking

Observed employee drinks on the prep table. Person in charge moved drink to a designated area.

Physical facilities not maintained in good repair & cleaned as often as necessary to keep them clean
