Upon acceptance of the permit issued by the regulatory authority, the permit holder in order to retain the permit shall: Post the permit in a location in the food establishment that is conspicuous to Consumers; Mount permanent business name to the outside of the establishment in a conspicuous location

Observed 2026 Food Permit to not be posted.

Using a Handwashing Sink - A handwashing sink is not maintained so that it is accessible at all times for employee use; is being used for purposes other than handwashing or is not being used in accordance with manufacturer's instructions

Observed basin of hand sink to the right of ice machine to be full of ice. Person in charge educated staff at time of inspection. ***REPEAT CRITICAL VIOLATION WILL BE ASSESSED IN 3-10 DAYS***

Package Integrity - Food packages were not in good condition and did not appear to protect the integrity of the contents so that the food is not exposed to adulteration or potential contaminants.

Observed 4 dented cans of caramel topping on can rack in back prep room. Person in charge removed at time of inspection.

Equipment & Food-Contact Cleaning Frequency - Equipment food-contact surfaces and utensils shall be cleaned according to the Food Code.

Observed build up on table top can opener blade in back prep room. Person in charge removed at time of inspection.

Floors/walls/ceilings not smooth & easily cleanable; and/or Utility lines unnecessarily exposed

Observed wall to the left when entering door dash room to be in disrepair.

Floor to wall junctures not covered; Water-flushed floors not provided with drains & graded to drain

Observed floor to wall juncture to be missing in the following areas: inner sides of door frame on right leading to back prep room. walls throughout and to the right of hot water heater nook.

Physical facilities not maintained in good repair & cleaned as often as necessary to keep them clean

Observed ceiling vent in right women's toilet room to be in disrepair. ***REPEAT VIOLATION WILL BE ASSESSED IN 3-10 DAYS*** Observed build up on ceiling vent in employee toilet room. Observed stained ceiling tiles through out main cook line area. As tiles become damaged they must be replaced with tiles that are smooth, durable, easily cleanable, non-absorbent, and light in color.

Hair restraints & clothes that cover body hair not worn by employees

Observed 2 of 5 employees working on cook line to have hair restraints.

Plumbing system not maintained in good repair

Observed drip leak from faucet over 3 compartment sink (non-critical).
