  Observed door gasket on white deep freezer to be in disrepair.

  Physical facilities not maintained in good repair & cleaned as often as necessary to keep them clean

  Observed build up on grated ceiling vent in front of front counter. Observed build up on ceiling vent and tiles in front of make table and soup well. Observed ceiling tile in front of closet ceiling light to entrance of kitchen to be in disrepair. As ceiling tiles become damaged they must be replace with tiles that are smooth, durable, easily cleanable, non-absorbent and light in color.
