  Physical facilities not maintained in good repair & cleaned as often as necessary to keep them clean

  Observed debris build-up on the floor underneath the soda bib shelves in the back storage area. Person in charge cleaned under the shelves at the time of inspection. Observed dust build-up on the ceiling vents above the reach-in cooler doors by the roller grills and inside the men's restroom. Person in charge cleaned the vents at the time of inspection.
