  Observed single use containers being used as scoops in bulk jasmine and long grain rice bins. Person in charge discarded and provided scoops with handles at time of inspection.

  Outside receptacles not designed with tight-fitting lids/doors/covers & properly installed

  Observed dumpster to not have lids. Person in charge stated dumpster is shared with adjoining apartments and property management removed lids.

  Floors/walls/ceilings not smooth & easily cleanable; and/or Utility lines unnecessarily exposed

  Observed missing ceiling tiles in front of sink in women's toilet room. Person in charge stated they had a leak (not active at time of inspection) and maintenance has been called to check for leak.

  Physical facilities not maintained in good repair & cleaned as often as necessary to keep them clean

  Observed build up on ceiling vents in front of expo line soup well.
