Observed dead flying insect on the prep counter on the left side of the hood vent. Person in charge removed at time of inspection.

Nonfood-contact surfaces not cleaned at frequency to prevent buildup of residue

Observed debris throughout the cabinet drawers and cabinets with pull-out shelving.

Plumbing system not maintained in good repair

Observed faucet of three-compartment sink to be loose [NON-CRITICAL]. Observed leak from faucet and cold water handle of the three-compartment sink [NON-CRITICAL].

Physical facilities not maintained in good repair & cleaned as often as necessary to keep them clean

Observed stained ceiling tiles throughout the kitchen. Observed broken and missing ceiling tiles throughout the kitchen. Observed dust buildup on the ceiling vents in the kitchen. Observed cove base falling off wall near the special occasion cabinets.
