Observed no soap available at the back or prep handwashing sinks. Person in charge replaced soap containers.

Hand Drying Provision - Each handwashing sink, or group of adjacent sinks, is not provided with Individual, disposable towels; a continuous towel system that supplies the user with a clean towel; a heated-air hand drying device; or a hand drying device that employs an air-knife system that delivers high velocity, pressurized air at ambient temperatures.

Observed no paper towels at the back or kitchen handwashing sinks. Person in charge replaced.

Observed buildup on the lobby soda fountain. Observed buildup on the lower shelf of the rear prep table. Observed buildup on the shelf near bread. Observed buildup on the floor of the walk-in freezer. Observed black buildup on the ceiling of the walk-in cooler. Observed buildup on shelving next to warewash three-compartment sink.

Physical facilities not maintained in good repair & cleaned as often as necessary to keep them clean

Observed grease and food debris on the floor between the fryers and reach-in freezer. Observed splatter on the ceiling tile above the middle warmers. Observed stained ceiling tiles over the reach-in freezer. Observed buildup on the wall above the three-compartment sink. Observed buildup on the wall above the produce prep sink. Observed debris on the floor below the drive-thru soda fountain.`

In-use utensils not properly stored between uses

Observed spoon stored in standing water at the frozen drink station. Person in charge removed.

When to Wash - Food Employees did not clean their hands and exposed portions of their arms according to Food Code guidelines.

Observed two employees to handle soiled dishes or touch a body part and proceed to put on gloves without washing hands first. Employees were stopped and instructed to wash hands; employees complied.
