Observed no biohazard clean up kit on site.Corrective Actions: Ensure to obtain a biohazard clean up kit with written instructions in case of vomit, diarrhea, blood spill, etc.

Consumer advisory provided for raw / undercooked food

Observed no allergen awareness poster.Corrective Actions: Ensure to post an allergen awareness poster in kitchen for staff.

Insects, rodents and animals not present

Observed 2 dead cockroaches in back corner of back room by deep freezers.Corrective Actions: Ensure to continue pest treatment with pest control company. Sweep up dead cockroaches, seal up possible entry ways at back door, and report findings with pest control company.

Wiping cloths; properly used and stored

Observed several damp wiping cloths being stored on prep tables and cutting boards.Corrective Actions: Ensure to store wiping cloths in sanitizer in between use. Manager stored cloths in sanitizer.

Food and non-food contact surfaces cleanable, properly designed, constructed and used

Observed one damaged gasket inside reach in cooler across from wok fryer.Corrective Actions: Ensure to repair/replace damaged cooler gasket.

Hot and cold water available; adequate pressure

Observed water was turned off for hand sink by 3-compartment sink at beginning of inspection.Corrective Actions: Ensure hand sinks are always supplied with hot water of 100F or greater. Manager turned water back on.

Physical facilities installed, maintained, and clean

Observed damaged bottom section of wall in hallway to bathroom, a damaged ceiling tile above 3-compartment sink, and damaged section of coving at entrance to back storage room. Observed air gap at back screen door.Corrective Actions: Ensure to make repairs to any damaged walls and replace ceiling tile. Ensure back door is completely sealed with no air gap present.
