  Observed front and back hand sinks were missing handwashing signage. Observed soap and paper towels for front hand sinks were missing were not easily accessible.Corrective Actions: Ensure to place signs at hand sinks to designate as hand sinks. Ensure soap and paper towels are easily accessible.

  Observed chili at hot steam well was not heated up to 165F for at least 15 seconds before placing in hot hold.Corrective Actions: Ensure hot hold food items are properly cooked to required temperature for required amount of time before placing in hot hold.

  Observed shredded cheese, jalapeños, relish, and onions at ice bath were past discard time and being held between 54-62F.Corrective Actions: Ensure to discard food items being tracked for time as a public health control according to written discard time. Manager discarded food items.

  Observed several floor and coping tiles throughout kitchen that were missing or damaged. Observed black mold-like particles on wall under soap dispenser in women's restroom. Observed build up of food debris and residue underneath shelves in walk in freezer.Corrective Actions: Ensure to repair/replace any missing or damaged floor/coping tiles. Ensure to maintain walls and floors clean to sight and touch.
