  Observed none of the veggies in either topside prep coolers are maintaining temperature of 41°F or colder. Temps are ranging from 48°F-56°F. Veggies have been in cooler less than four hours.Corrective Actions: Employee voluntarily moved all ingredients into large reach in cooler. Work order is in for both coolers. Ensure cold hold ingredients are maintaining temp of 41°F or colder.

  Observed what appears to be either salt or sugar in an unlabeled bin above prep sink.Corrective Actions: Ensure to label all products if not easily identified.

  Observed sanitizer below 150ppm at hand sink across from prep line. Corrective Actions: Ensure quaternary ammonium concentration is maintained between 150-400ppm.

  Observed scoop for cut green onions stored with handle touching food product.Corrective Actions: Ensure scoops are stored so handles are not touching food products.
