Observed several damaged floor tiles throughout the back area and significant damage by the back door.Observed the frp is loose by the back door and the coving is not fully sealed to the wall. Corrective Actions: Ensure the damaged tiles are repaired/replaced. Ensure the coving and the frp is fully sealed to the wall.

Observed empoyee food stored on the same shelf of a prep table as restaurant food contactsurfaces.Corrective Actions: Ensure employee items are stored below and away from restaurant items.
