Observed milk in the walk in fridge with an internal temperature of 45*F.Corrective Actions: Ensure all temperature as a control for safety foods are kept at 41*F or lower.

Insects, rodents and animals not present

Observed a dead mouse on a sticky trap and rodent droppings in the cabinets under the microwave and coffee machine. Corrective Actions: Ensure to discuss a pest management plan with the pest control company. Ensure to have a pest control service within the next 7 days. Ensure a licensed pest control operator provides all pest control services. Ensure to clean under the cabinets. Ensure establishment is cleaned nightly and all surfaces are resanitized every morning.

Observed the ice scoop did not have a handle and there were no gloves on site.Corrective Actions: Ensure to use an ice scoop with a handle to ensure there is no bare hand contact with the ice.

Observed the walk in cooler is unclean on the ceiling, shelving, and floors.Corrective Actions: Ensure the walk in cooler is cleaned regularly.

Observed the drain tube for the ice machine and the 3 compartment sink had an insufficient air gap.Corrective Actions: Ensure the drain tube for the ice machine and the 3 compartment sink have a sufficient air gap.

Observed several ceiling tiles have water damage and holes.Corrective Actions: Ensure the ceiling tiles are repaired or replaced.

Observed the light in the back room with the ice machine is not working properly and the room is too dark. Observed the light covers have debris throughout the establishmentCorrective Actions: Ensure the light in the back room is repaired. Ensure all the light covers are cleaned.
