  Observed no soap at the designated hand sink at the bar. Observed employee using the bar hand sink as a dump sink. Observed hand sink in the back ware washing area blocked by a cart and with no paper towels.Corrective Actions: Ensure to have all designated hand sinks supplied with soap and paper towels. Ensure all hand sinks are easily accessible.

  Observed a couple live roaches near the floor drains in the back ware washing area. Manager provided recent pest control records from within the last month and states they are scheduled for treatment soon.Corrective Actions: Ensure pest control is treating facility for roaches. Manager will have pest control treat for cockroaches before follow up next week.

  Observed personal items on top of in use cutting boards.Corrective Actions: Ensure personal items are stored away from food prep areas.

  Observed various silverware utensils stored facing upwards.Corrective Actions: Ensure utensils are face down to prevent contamination.

  Observed what appeared to be black grime on the upper interior of ice machine.Corrective Actions: Ensure to properly drain, clean and sanitize ice machine.

  Observed improper air gap below the bar hand sink and below the hand sink in the back located near the exit door.Corrective Actions: Ensure to create adequate air gap to prevent backflow.

  Observed fluorescent lights missing protective covering in the walk in cooler and freezer. Observed missing protective coverings for 2 lights in the fume hood in the back area of kitchen.Corrective Actions: Ensure all glass lights have a protective covering.
