  Observed missing tiles throughout the facility. Obsrved deep crevices in the coving and the floor. Observed water pooling outside the walk in door. Observed the walls are discolored and not cleanable to sight. Observed the hand sink by the dish machine splashes onto the clean equipment shelves.Corrective Actions: Ensure the missing tiles are replaced. Ensure the coving is sealed. Ensure all surfaces are clean to sight and easily cleanable. Ensure the hand sink has a splash guard.
