  Hands clean and properly washed

  Observed an employee put on gloves without washing hands. Observed an employee wash hands for less than 20 seconds and then turn off the faucet with their bare hands. Corrective Actions: Ensure employees wash hands prior to putting on gloves. Ensure employees wash hands with soap and 100*F hot water for 20 seconds, dry hands with a paper towel, and turn the faucet off with the paper towel to prevent recontamination.

  Observed the serving trays with the food contact paper on them next to the hand sink under the paper towels. Serving trays were moved immediately.Corrective Actions: Ensure the serving trays are protected from contamination at all times. Ensure all food contact surfaces are protected from contamination.

  Observed several holes in the walls.Corrective Actions: Ensure all holes in the walls are sealed to prevent pest and to ensure walls are smooth, durable,nonabsorbant, and easily cleanable.
