Observed several boxes of chips and cereal stored on the floor in dry storage area.Corrective Actions: Ensure all food items are stored at least six inches off the floor.

Personal cleanliness

Observed several personal items stored on prep tables during food preparation. Observed employee food items stored above/next to food items that are served to students in walk-in cooler.Corrective Actions: Ensure all personal items are stored in the area that is designated for employee belongings. Ensure all employee food items are stored below/away from food items served to students.

Single-use/single-serve articles; properly stored, used

Observed single-use trays stored with food contact surface facing upward on top of left milk cooler.Corrective Actions: Ensure all single-use items are stored facing downward.

Adequate ventilation and lighting; designated areas used

Observed five out of six light bulbs out under vent hood.Corrective Actions: Ensure to replace light bulbs under vent hood to provide adequate lighting.
