Observed sanitizer solution in 3-compartment sink testing between 0-100 ppm.Corrective Actions: Ensure sanitizer solution is maintained at the required concentration of 200-400 ppm.

Observed right standing cooler next to manager's desk not properly functioning with an ambient temperature of 98°F.Corrective Actions: Ensure to repair/replace cooler.

Observed employee food items stored above/next to food items that are served to students in left standing cooler next to manager's desk.Corrective Actions: Ensure to store employee food items below/away from food items that are served to students.

Observed dish machine not functioning and being used as storage.Corrective Actions: Ensure to repair/replace dish machine and remove items that are currently stored inside.

Plumbing installed; proper backflow devices

Observed 3-compartment sink has a direct connection with the sewer line.Corrective Actions: Ensure to provide an approved air gap to prevent sewage backup or contamination.

Physical facilities installed, maintained, and clean

Observed chipped paint near vent hood and on walls all throughout kitchen.Corrective Actions: Ensure to repair/repaint affected wall surfaces to provide a smooth, durable, easily cleanable surface.
